Clinical Trial into Puberty Blockers on Hold After Medicines Regulator Steps In (2026)

The UK's clinical trial into puberty blockers for children has been paused after the Medicines and Healthcare products Regulatory Agency (MHRA) stepped in, citing concerns over the 'unquantified risk' of 'long-term biological harms'. The trial, known as the Pathways clinical trial, was set to begin recruiting participants aged 10 to 11, but the MHRA has now recommended a minimum age limit of 14. This decision comes after the Cass review, which found the evidence for the benefits of puberty blockers in children with gender dysphoria to be 'poor'. Dr Hilary Cass, who led the review, emphasized the need for a trial to address the passionate beliefs of clinicians, children, and families in the medication's benefits. The Department of Health and Social Care (DHSC) has stated that the safety and well-being of the children and young people involved are the top priorities, and the trial's preparations have been paused while the MHRA and clinical leaders address these concerns. The Pathways trial aims to recruit 226 young people over three years, with the youngest participants initially set to be 10 to 11 years old. However, the MHRA's letter highlights the potential for significant, unquantified long-term biological risks and suggests a graded approach, starting with those aged 14 as the lower limit of eligibility. This pause in the trial reflects the regulatory process's focus on safety and the need for rigorous scientific evidence to support such complex medical interventions.
